三、 具結書正本1份。

四、 授權書正本1份。
One original copy of Authorization Statement

五、 戶口名簿、護照或其他國籍證明文件影本1份。
One photocopy of family book, passport or other proof of nationality

六、 財力證明(具備足夠在臺就學之財力證明書3000美金以上，限提供本人或三等親(父母、祖父母、兄弟姊妹)出具財力證明、匯款證明；非本人之財力須另繳附親屬關係證明。)，或政府、本校或民間機構提供全額獎助學金之證明。
Financial Proof (Proof of sufficient funds for studying in Taiwan, with a financial statement of USD 3,000 or more. Only financial statements from the applicant, parents, or third-degree relatives (parents, grandparents, siblings) are accepted, along with remittance proof. If the financial proof is not from the applicant, additional proof of family relationship is required. Alternatively, proof of full scholarship provided by the government, the university, or private organizations is also accepted.

七、語言能力門檻
1.申請入學中文授課班級者，華語文能力測驗(TOCFL)基礎級(Level 2，相當於 CEFR A2)聽、讀 2 項皆須達 A2 級(含)以上(入學第 2 年須達 B1 級(含)以上)。
2.申請入學英文授課班級者，英文能力須達 CEFR B1 級(含)以上(入學第 2 年華語文能力測驗(TOCFL) 基礎級(Level 2，相當於 CEFR A2)聽、讀 2 項皆須達 A2 級(含)以上)。
Language Requirement
1.For applicants applying to Chinese-taught classes, proficiency in Chinese language as measured by the Test of Chinese as a Foreign Language (TOCFL) at the Basic Level (Level 2, equivalent to CEFR A2) is required, with a minimum of A2 level achieved in both listening and reading (B1 level or above is required in the second year of enrollment).
2.For applicants applying to English-taught classes, proficiency in English language must be at the CEFR B1 level or above (TOCFL Basic Level (Level 2, equivalent to CEFR A2) with a minimum of A2 level achieved in both listening and reading is required in the second year of enrollment).

八、申請入學身份切結書

九、最高學歷或同等學力證明文件1份（中、英文以外之語文，應加附中文或英文譯本1份）。
One photocopy of the applicant’s graduation diploma or degree certificate (These documents should be translated into English or Chinese if they are in other languages).

十、歷年成績單證明文件1份(中、英文以外之語文，應附中文或英文譯本)。
One photocopy of the applicant’s official academic record (These documents should be translated into English or Chinese if they are in other languages).

十一、學生與校方新型專班合約書一份
A student/school intense program contract

十二、學生與企業新型專班合約書一份
A student/company intense program contract

十三、自傳1000字以內（選繳）。
Autobiography (under 1000 words, * optional)

十四、各項證明能力之文件。
